Love Ambassador Nomination Form

To be considered a Love Ambassador, individuals must strive to be an unconditionally loving in their daily lives. If you know someone like this or you are this type of individual, then we want to hear from you. To nominate yourself or another person for this recognition, please fill out the form below or send us the information by mail.

Please include a vision statement explaining how you or this person embodies love along with a brief bio (biography), resume or CV (Curriculum Vitae), of the nominee if possible. Include any substantiating documentation or web pages that support the nomination. We also need your complete contact information, as well as theirs, so that we may send an acknowledgement if accepted.

Each nomination is thoroughly considered based on all the information provided. Reviews are processed approximately every other month and those accepted are notified via email.
